on Saturday oct. 24, 2004 around 3:45 pm my co-worker and myself took trash out to the comercial bin out back. we found not one box, but tons of boxes the entire commercial trash bin was full of charts. I pulled out one of the files to make sure that it did not contain any patient information. I found Patient Social Security numbers and patient private information.

this is where i also founf the doctors referral acknowledgement form with his name address and phone number. we called his office to let them know that in accordance with the HIPPA [patient privacy law] they needed to come back to pick up the charts because the information by law needs to be Shreded. Please investigate.
